To sign up for the 1000 Club:

1. Fill out the application form below with the name and address – or of the gift receipt, if the membership isn’t for you.

2. Write out a check for $55 (dues for a year) payable to: Sacred Heart 1000 Club. If you prefer, you may send monthly payments of $5 per number instead.
